Request for Quotations (mailed or picked up) should come with a cover, general note sheet, instructions for the bid, the listed specifications with estimated quantities and a summary sheet.
- The cover should contain the name, address and contact number for the Owner as well as the name, address and contact person and number of issuing agency.
- The general note sheet contains information on the purpose for the bid, responsibilities of the bidder and direction on how fill the bid.
- Instructions are given to how the bid sheets are to be handled and restrict their alteration, how and where to submit the bid, due date and associated instructions.
- Specifications with quantities should have a space to write the quoted price.
- Summary sheet should list the bid specifications and their collective total price and number of days to complete all of the items.
